Disease management programs
UnitedHealthcare® disease management programs are designed to help members with chronic conditions, such as diabetes or certain types of heart disease, to be their healthiest. UnitedHealthcare offers education and resources to support optimal health of people who are currently being treated for chronic diseases. Members receive case management and can attend workshops on how to manage their disease with confidence.
Care coordination programs
These programs support the decisions made by plan members and their doctors and help promote successful treatment. For example:
- Before a hospital visit: Care coordinators provide admission counseling to explain diagnosis, treatment choices and expectations during the hospital stay.
- During a hospital stay: Inpatient care advocates help make sure the doctor's orders and treatment plans are carried out as directed.
- Home from the hospital: The Welcome Home! program helps plan members with their recovery needs so they can focus on getting well.
Plan members also receive Healthy Reminders in the mail to encourage preventive health. If a member's health care conditions become more complex or chronic, resources are available for additional education, information and health care support.
