Eligibility - Medicare Advantage Plans
If you are new to Medicare, turning 65, retiring, moving, qualify for assistance, or have another qualifying situation, you may be eligible to enroll in or change your plan. A Medicare prescription drug plan may provide the coverage you are looking for. For all others, Medicare Open Enrollment is October 15 - December 7. Learn more about eligibility and when to enroll.
Enroll in a Medicare Prescription Drug Plan
To enroll by phone, you can call toll-free at 1-877-596-3258 (TTY 711), 8 a.m.–8 p.m. local time, 7 days a week to speak to a representative.
To enroll by mail, download and fill out the enrollment form, then print it and mail your completed enrollment form to UnitedHealthcare.

You will need information found on your Medicare card to enroll online.
Eligibility - Medicare Special Needs Plans
To enroll in a Special Needs Plan you must have Medicare Part A and Part B, live in the plan's service area, and meet the plan's specific eligibility requirements, such as one of the following:
- Qualify for both Medicare and Medicaid (also referred to as "dual eligible" ).
- Live in a long-term care facility such as a contracted skilled nursing facility.
- Live in an assisted living facility and need the same level of care as people who live in a long-term care facility, like a skilled nursing facility.
- Have one or more specific chronic or disabling conditions, like diabetes, chronic heart failure or specific cardiovascular disorders.
